    I've simulated both experiences dozens of times on multiple clients, but I am unable to replicate this. There is nothing in the code that should cause this, so it seems more likely to be a engine bug, and possibly one that has already been patched.

    I struggled to replicate this, but I think the issue is fixed. If it occurs again please post any interactions with weapons prior to the issue.
