You take a picture of your character ingame, or get someone to take one for you.
You could get a friend or just someone in general to take a picture of you infront of a greenscreen to make it easy for you then edit the picture in whatever way you want while still following the rules.

You can either get another player to take a screenshot or photo of your character, or you could go up to the Jennifer's mirrors, or the bathroom at casino(I believe there is a mirror there)

I recommend taking a picture of your Character in front of a TV running a greenscreen, so you can easily remove the background and make it transparent.

Another solution that you could do is to recreate the character in GMOD single player or in SFM (Once you've extracted the content over) in a manner to how I've done with my current profile picture.

If Tooter still works you can also take a "selfie" and use the icon it generates but it's probably low res, don't quite remember.
