Map Bug Report (Spawned dead upon server restart)

Type of Bug: Map

Description of the Bug: I was in the slums 4 bathroom and after server restart I spawned dead (Red Screen) outside the slums apartments near the fire department with my gun completely gone.

How to reproduce the Bug: Unknown. I was in the slums 4 bathroom before this happened.

I edited this to include more information.

To my best knowledge this seems to be an attempted debug from the server as when he spawned back in he was teleported right outside the bathroom which caused him to fall and die.
