Most likely not the correct file format or sample rate. Make sure your file name and folder names are all correct and aren't being prioritised over other addons for some reason so disable unecessarry addons. Make sure to do snd_restart in console if you're modifying these files while you're still in-game.

Can we see your filepath to make sure it's not wrong?
