Community Survey

Been a while since we've had any major community interaction so Tyla and myself have created another Community Survey, we haven't had one since January and whilst we did have a Community Meeting in between, I personally found the survey more useful for identifying issues.

The compulsory section is simply for filling in your details and feedback on the usefulness on the survey. All other parts are optional, so if you only care about PD you can just fill out PD section and if you only care about Administration, you can only fill out that section.
