You’ll need to disable/uninstall the first 4 addons. I also choose to disable all addons besides the map and content when playing to avoid any conflict, but it’s preference I guess.

In case you still need to keep other addons from other servers where it conflicts with perp content, follow this guide:

Conflicting Content Fix

So, Most of you probably have content from other servers which then conflicts with PERPHeads. I have a solution for you. We're going to use something called 'Presets'. This will allow you to load it from the addons section of GMOD and disable the others. This will keep all addons installed but...

This will allow you to keep other addons but not worry about having them conflict. Simple fix to do and doesn't take too long.

@flugs has managed to fix this for you and the above thread is designed to just make "Presets" that allow only content in those presets to load.
