Delete whole gmod folder,
Download gmod again
First fetch Css folder
Than put Perpheads pack from above in the folder
Than join server and it should be fixed

Are you 100% sure that you put '-dxlevel 90' in the launch options? That has fixed it for quite a lot of people who had this issue.

get CSS and half life ep 2 textures

have you mounted your CSS and half life 2 games, when in gmod main menu. click on the controller and tock all boxes available and restart gmod
