How does ban count work?

Depends. If you have 1 ban which is a year old, then it won't get the same punishment as getting it from last week.
Having a year old ban and not even having one warning about it is like saying you tried your best over the past few year but then you did a mistake once in a year

It's up to the staff member's discretion. E.g, a ban from perhaps six months ago won't be taken much into consideration, unless the rule infridgement can somehow be close to the infridgement you've recently done. E.g breaking the same rule, aswell as considering your activity, any patterns concerning your record, and so on.
